Flu Vaccine

    1. Please print, fill out, and bring this CONSENT FORM for each person receiving a flu vaccine. Payment and charges: Make sure our office has updated insurance information and a credit card on file prior to your flu vaccine appointment. There is no out-of-pocket cost for our patients with insurance. For parents receiving the flu vaccine, we do not bill insurance therefore the out-of-pocket cost is $35 for flu shots and $40 for flu mist due at time of injection. These charges apply to self-pay patients as well.If your child is sick (fever of 100.4 F or above), waiting on COVID19 test results, or quarantining for COVID19 exposure, please cancel and reschedule the flu vaccine appointment.
    2. Flu vaccine with well checks: We offer flu vaccine at scheduled well check appointments for the patient and family members in attendance. 
    3. Flu vaccine with ‘sick’ visits: We will offer flu vaccine to patients who have appointments for non-infectious sick visits (eczema, ADD medication checks, etc) as well as family members in attendance.
    4. Flu vaccine for tag-a-long siblings: If you are bringing one child in for a well check and want a sibling to come along and get a flu vaccine that day, please notify the office ahead of time and print/fill out the flu CONSENT FORM  to bring to the visit.
    5. Flu vaccine eligibility details:

—Children ages 6 months and up are eligible to receive a flu vaccine injection.

—Children under 9 years of age receiving flu vaccine for the first time need 2 doses, given at least 4 weeks apart for optimal protection.

—Healthy people, non-pregnant people ages 2 years—49 years old can receive Flumist nasal spray. Children with a history of asthma or wheezing in the past 12 months are not eligible for Flumist.

RSV protection (Beyfortus)–

We will receive our first shipment of Beyfortus in the coming weeks. Please call the office if your baby is eligible to get it and you are interested in it.

Beyfortus is a 1-dose preventative monoclonal antibody shot that gives a baby passive immunity without having to mount an immune response like a vaccine. This medication came out last year and significantly reduced the incidence and severity of RSV infection in babies. 

Beyfortus is recommended for:

> All babies 0-8 months of age who are born in the fall through the spring and the mother did not receive RSV vaccine during pregnancy (or was born less than 14 days after maternal RSV vaccination)

>High risk children ages 8 through 19 month with chronic lung disease of prematurity or are immunecompromised.

>Most babies 0-8 months of age whose mothers DID receive RSV vaccine during pregnancy are NOT eligible for Beyfortus.
