Transferring Medical Records

New to Austin? Changing providers?

We welcome you and look forward to meeting you and your family! We kindly ask you to provide your child’s current vaccination record and growth charts before making your first appointment with the practice. Ideally, we would like to have your child’s complete medical record prior to the first office visit, especially if it is a consultation or second opinion type of visit. We have this policy in place to ensure the most appropriate care and to give you an efficient, positive experience at your first visit.

To transfer your child’s medical records, just print and fill out an authorization for medical records release. You will need to send that form to your current medical provider’s office, where the records are housed, and that office will send the records to our office.
